Maybe something is bothering him thats not so crazy, but more of a normal type of thing. Don't drive yourself crazy with it. Just make sure he gets to talk to you every day and you may be able to pick up on something, or not.

did he ever make happy faces? maybe like mandy (I think) said, he doesn't know how to make happy faces. if it was a sudden thing I would be more concerned.

I agree with the above, you know how many kids make thier S's backward maybe he has his smiles backwards, did you ever ask him why he does that? you'd be surprised what they say when just asked about a certain behavior. he might just say "I think the sad face is funny on drawings" you never know

any idea, after all the therapy, why he was this way at this young age? and it sounds like therapy was a failure after what you wrote here and in the Teen thread

it wasn't a failure as much as it got the hanhala to let him back in to yeshiva; but it didn't change him at all.

I would say why don't you take him to a psycologist. You want to make sure there are no long term things Chas V'shalom going on!
