DD has had a heat rash (who hasnt!) because of this unrelenting heat. Shes had it since last week. It starts on her neck and spreads down, front and back, to mid torso, but the higher concentration of rash is higher up.
We dont have central air. What should I do? Do I need to call a dr?

Powder her up with baby goldbond , it will help the rash. do it in the morning before she sweats.

One of my twins (the chunkier one  ) has also had an awful heat rash the past few weeks.

when you say theres no central air, do you mean that the babys room has no air conditioner also? if so, I would invest in one. I think the rash gets worst at nighttime.

My baby also has it - on her upper arms and upper thighs. She has lots of delicious chubbyness there so I'm guessing that is part of the cause. During the day she is pretty cool and at night she is in my room which is very cool at night. I'm not sure what to do about it.

all you can do, is keep the kids cool and from scratching. we used an anti itch cream.
