First I would like to apologize for another vaccination post. However I am very curious what you do about school medical forms. I know if I am not up to date on even one shot I get hassled, so what do you gals do?

I know my rights, and exercise them. that's all.
the onus of vaccination is not upon the parent. The school must show proof to the dept of health (or the BOE, I'm not sure) that each student is either fully vaccinated or legally exempt. so when you are not up to date, it causes a problem for the school, I believe they can be pretty stiffly fined if they are found to be accepting students who do not meet the above criteria, which is why they hassle the parents to get the paperwork in on time.
my paperwork is always in on time and in order, even if my kids dont have all the vaccinations. I just file exemptions for the ones we skip.
