How to address Midwife?

I am writing a thank you letter to a midwife who helped me- how do I address her in the note?

She is not a doctor, she is a CNM. She is married, so I write "Dear Mrs. X"?

I dealt with her once, we are not on a first-name basis.

Any midwife I have ever delt with (and that's a LOT) have all gone by first names.

Same here. I don't understand-- how does she introduce herself? Or when you call to make an appointment, what does the scheduling person call her? I would think if she doesn't want women to call her by her first name (which is extremely unusual for a midwife) she would let them know how she wants to be addressed.

I call everyone madame (mrs) or mademoiselle (miss) if very young looking at hospital. Doctors, if I know they are doctors, I will call doctor generally. LOL. But they normally have their name on their blouse (my English is slowly escaping me...) and you def can call them like this.
