Child health plus renewal; no proof of income?

I'm renewing my daughter's child health plus but there has been a drastic change in my income. I lost most of it. My only income currently comes from seasonal free lance work which is usually paid in cash, just because the amounts are small. I need to show income, and I'm not looking to hide anything but I have no proof of this income and I have no idea what to put down. I called child health plus to ask but they said they would send someone out to my home to do the paperwork.I don't want anyone coming here. I just have this one question.

My husband works with computers and does a lot of odd jobs through different agencies. He doesnt have an 'employer'. When we applied for CHP/FHP we were told to include a monthly 'profit and loss statement' going back 3 months. That gives you a chance to show variable incomes for different months, and allows you to show what business expenses eat away at that total. Im sure if you google it, you can get a template.
