Small orange/brown stains on white shirts after wash

Sometimes, white shirts come out of the washing machine with small orangy/brown stains on. They're usually very small - biggest maybe 5mm in diameter - sometimes they are just dots. They usually come out clean if I re-wash the shirt with a tiny bit of bleach on the stain. Does anyone know what causes this?
