I'm giving a gift to a doctor who has given a significant amount of care without charging.

When and how do I give the gift? At the next appointment directly to him? To his staff to give to him? Should I send it via mail?

Any advice on protocol would be appreciated.

I know my parents used to give for a few doctors that helped them a lot, on their new year every year

I usually put it in a pretty bag and give it after the visit, or I put it into his office and let him know that I did. I usually take some chocolates to the nursing staff too, so I dont feel weird giving something to doc only.

As it's practical or easier. Not everything needs a strong protocol Smile

Is this a local/frum doctor? You can always have it delivered to their house.

Otherwise, I would have it delivered to the office.

Personally, I always find it awkward to go through the whole 'You shouldn't have!' business. So, delivery is always best. The only thing I do myself is a 'gift' to my OB's office -- I bring in coffee and donuts from Dunkin Donuts (which is right near their office, so it isn't a big deal) at my yearly and post-partum. I think that when a staff knows you that intimately (even if what they are doing is 'routine') that it's best just for them to have a nice feeling when they think of you, if that makes sense.
