I work every day including Friday, and I work it out in different ways- evening appointments, taking off sometimes, I go whenever I have a day off or my husband has off, etc.
But, I just want to say, that if someone asked me to watch their kids so they can go to the doctor, if I could easily do it I would say sure. If it would be too difficult or for whatever reason it wouldn't work out, I would say no. It's pretty simple on your end.
You don't have to try to figure out her schedule or when she should go. If you don't want to watch her kids, tell her you can't. If it's not a big deal, then do the favor for her. And if you don't want to set a pattern, then just say yes once in a while.

Having no money does not mean she is stingy.
(Not that she should rely on favors, just the assumption bothered me)
