If someone wanted to steal they need a lot more than your signature. She is not asking for your social proving that she works for you.
I would ask her to bring in the medical form for financial assistance so you can verify that she has a legit need and then write it for her.

I am not worried so much that she is going to forge OP's signature. I would worry that she wanted to collect money from OP.

I'd start asking her for proof of payments, a receipt each time she gets paid.
