I received a bar mitzvah invitation from overseas. This family never sent a gift for any of my sons' bar mitzvahs, although I sent them invitations.
If I'm nice and send a gift, won't that be kind of insulting, like saying that I'm better than them?

If you are in the U.S. and they are in Israel, then you can send a gift if you want to, and they'll appreciate it and not feel badly that they didn't send you one.
And if you are in Israel and they are in the US, you don't need to send a gift. A nice card is really enough, unless you are in contact by facebook. Then that's enough.

Your policy of sending gifts when you can't attend something should have nothing to do with the recipient (at least in this scenario). You do what you feel is right--don't worry about offending the family. I think you're coming off as thoughtful!!

A phone call to say mazel tov might even be nicer than a gift. But I don't think you can go wrong with sending a gift. Some people are just more gifty than others.
