His bed has a waterproof mattress protector under the sheet. Still, the urine leaked through ( ) and now his mattress has a huge wet, stinky spot. How can I get the smell out?

I spray Febreze on the spot and let it air out. It takes away the smell, although the Febreze itself gives off a chemical smell (which is why I let it air out).

Nature's Miracle. Its for pet odors, but it works for human as well.

I mix vinegar and water in a spray bottle, spray liberally, and let air dry.
If there are remaining stains you can clean with bleach water.
I don't feel like febreeze actually removes any odors, but the vinegar will for sure.

Pour vinegar on the stain. Take a towel and stand on it to absorb as much back out as you can. Sprinkle with baking soda and let dry. Vacuum the mattress.

Let a generous amount of baking soda sit on the stain for a while. Vaccum it away the odor should be gone. Though u might still see the outline of the stain...

Soak with seltzer first. Let dry. Then sprinkle baking soda on, let it stay for a while, then vacuum.
The smell of Febreeze gets me sick.
