Hi- I need to apply my child for Child Health Plus and would like to know what the required documents are and if it can be done online.

There's so much red tape. You can lose out by doing it yourself and by mistake leaving something out and losing out on full month of coverage or worse. Make an appointment with an agency. They're so helpful.
