If you knew your child had significant social issues or ADD or other struggles, did you start therapy at 3? Or did you wait until they were school age and it became a more noticeable issue? Is waiting until 6 better or if you already see clear struggles, did you start before school age?

It depends what kind of therapy
It's hard to answer because a lot of issues are not diagnosable at age 3.
OTOH, you could help your child out by having play dates and teaching him proper social interaction.
Research what you can do at home for him.

If there is something you can see your child needs help with, and/or they have a diagnosis, then advocate for therapy right away. The earlier you address issues the more progress you'll see.

I don't see how there would be a benefit in waiting, as the earlier you start therapy the better. Social skills are hard to diagnose at three years old, so that particular issue may not need to be addressed yet.

But a full scale evaluation would pick up any other issues, which, from the op's description it seems that there are, and therapy can be started immediately, if possible.

The earlier the better. A kid with issues at 3 has no self esteem issues attached to the handicap. A kid with issues at 6 already realizes they are different and feel bad about themselves. Also, the younger the kid, the more plastic the brain and the more easy to change the brain. at 4 there is a certain level of plasticity lost, at 9 again, and at 26 for the last time. The most ideal time is to be done with therapy before 4. My child was in psychotherapy and OT from age 3-6.
