Can you help diagnose this rash?

My cousin writes:

The rash is just a little puffy. It feels smooth. She doesn't itch or seem to notice at all. The allergist describes it as having a "target pattern" with pink/red in the middle, then clear, then a red outline. She said the most common issue would be that a virus is causing it, but it wouldn't go away and then come back like hers has, and it would have cleared up by now.

Her skin issue is unresponsive to the antihistamine and most of her tests are negative. (Her supposed nut allergy is not causing this and never was. Sigh.) Allergy has now referred us back to the dermatologist to rethink his hives diagnosis.

Could be viral hives. I forgot the exact term the allergist used.
The hives can come and go for a few months. They can also be worse evening and morning time, but not always.

Was she in high grass, or an area that may have ticks? Their bites look like bull's eyes.

That sounds like what a tic rash looks like. Check for tics.

When my allergic son broke out in hives both the allergist and ped said it was most likely viral and something like 80% of hives are viral. Can take several days to go away.

Ringworm? Although when my daughter had ringworm, she didn't have so many splotches, only one. I second the idea to consult the pediatrician. He/She may have seen this before.
