So I am currently enrolled in a community health class for my bachelors.
We have to write a community health analysis of a community.
My topic is access to healthcare within the orthodox community of Los Angeles. It has to be Los Angeles, because I reside in Los Angeles. The only way I could not go crazy is to focus on a particular group within Los Angeles, I.e. the orthodox community.
I created this survey on Survey monkey, if people could please give me feed back so that I can improve the survey or just respond to the survey. I would appreciate it!!

FYI - can't actually take the survey.
edited to say that it did work on the second try, must be something at my end. Also, you may want to make the three part question into separate queries.
