toys - no such thing as too many of those, and when the babies are 4+ months it's nice to have a variety of baby toys they can play with. I love the Lamaze brand of toys their products are so cute. Books are also another nice gift if you know she has enough clothes and things.

A baby carrier, if she doesn't already have one. Often it's when you have the second kid that you experience a need. Let her choose which type she wants.

What's the budget?
I actually asked my sister this morning if she would outfit the baby for Yom tov - on pesach. She wants to spend a lot of money and these things are more important to her than me (I generally can't stand to spend $20 on a stretchie from gap) so the practical of it (he will need 'real' clothes months from now) makes me happy and the quality will make her happy.
