I think the answer should include family size/situation:
Once a week, we're a young couple.
Every second week, we're a family of five and I work part-time.
Once a month. Family of eight and work full time. Don't judge.
...
ETA: FTR, my answer would be the second option above - approximately every two weeks - though my actual family size is slightly bigger.

OMG I am so embarrased to post this.
1-3 times a year maybe. Definitely before Pesach and for a new baby.
I put my white sheet on top of the colored one.
I have a bunch of kids but am very overwhelmed when it comes to housework. No clue what I'm doing. I feel so pathetic looking at the answers here.

Don't be embarrassed. don't let other people shame you. None of us are perfect, we all have flaws. You and I, we have similar flaws. I'd imagine that you, like me, try every day to be tidier. I fail on a daily basis, but the thought it there. fist-bumping to you, amother, there are far worse things than this.

Weekly because we have sensitive skin and allergies.
First couple of years of our marriage we changed the linen every other week.

I try to remember every few weeks but sometimes it ends up every other month. I dont keep track. Duvet covers get cleaned for pesach. I also just put a white sheet on top of colored one.
Its such a pain to reach the corners to put new sheets on and I dont have cleaning help.

Used to change every 2-3 weeks. Now I'm trying for every week/10 days because of allergies.

Don't your sheets get dirty? You must change it more then 3x a year!! I wish I could change my linen not as often but with the kids and time with my husband, it doesn't stay clean that long. And I change out the pillow cases more often then the sheet and blanket. There's nothing like going to sleep on freshly washed linen.

Orig amother, I'm the fist-bumper. I bathe daily, I rotate pillows in and out, and I wear PJs.
Other people need not tell us we are gross.

I sort of put up with the dirt I guess.
husband time - I'm not changing the linens after every time, and a bunch of stains doesn't seem so different as one - it honestly never occurred to me to change bc of that
kids - they bring crumbs with them and I brush them off. If there's serious spit up I will change the linens but that doesn't happen so often.
