Anyone know of seminaries outside of NY or lakewood other than Yavneh, Toronto or Montreal (Yes, I know these are not in the US.)

There is one in Detroit, but I don't know anything about it (assuming it still exists).
In Chicago, it really isn't a seminary, but possibly a seminary alternative (though most girls go post seminary) -- TI which is a fully certified college program in many areas with required high level limudei kodesh as well (affiliated with Hebrew Theological College/Skokie Yeshiva).

Maalot Baltimore does not have a 1st year seminary program this year due to lack of enrollment. I don't know what their plans are for next year. They do have a fantastic and highly successful 2nd year program.

Yavne in Cleveland.
Didn't Montreal used to have a seminary (I know it's not the US, but close....)

Yes, but OP said not Yavne or Montreal. I stand corrected about Denver.

I think Montreal still has a seminary.
No, there are not a lot of chutz la'aretz options. I have to say that Rebbetzin Assaf's seminary looks interesting, but that's in NY so won't help OP.
