My mother has a (2nd) cousin who has been a grandmother since I've known her, she's probably in her mid-60s by now I.e at least 2x my age, I used to go to her house for Shabbos when I was single she has kids within 10 years of my age. We've never been "close" but I have a reason to call her, and I've never been sure how to address her. What would you do?

We call all relatives, close or distant, old or young, by their names. Nothing else.
Why the need for anything other than that? unless she asks you to call her auntie something or whatever title she is to you?

Title is so much nicer, more respectful, gentler.
Aunt/tata/doda/tati/tante/tanta... or cousin, but that may be more antiquated.

That's what I did when I was a kid for my mother's friends, but we're not that close, that's what aunt/doda/tante implies.
I guess I always felt weird calling her by her name (I always just avoided it when I was at her house, and when I would call I would just say, "hi, this (me), how are you?"
