What number does one use for self-employment income when applying or renewing for government programs? Current month's income? Average for the past 3 months? Estimated annual net income divided into 12?

This is a new financial situation, so I can't just use last year's numbers.

I get paid 1099 from a company that I work for. I renewed my medicaid last year and I was told by nachas in boro park to bring a letter from the company stating the amount they paid me the last 2 or 3 months...
