Machsom L'fi (Netzur L'shoincha) (what to say before)

I remember seeing on someones fridge a little paragraph one should say before doing a machsom l'fi either a yehi ratzon or just I hereby take upon myself to not speak lashon horah during these hours for a zechus refuah sheleima. I was able to find something in English but I've seen something that was in lashon kodesh.

If anyone has this paragraph and can share it with me please let me know. My family would like to do such a thing for a sick family member but I can't find it anywhere nor remember who's fridge it was on.

This is all I found from doing an online search.

I will refrain from speaking and listening to, reading, and writing, loshon hora from: ___ to: ___ for the merit of ___________."
