I have it... annual dr visits and then there is bonuses, but I haven't seen the bonuses yet. My postpartum visit with my midwife showed up as my annual visit so not necessarily does it mean a regular doctor.

It is a prepaid debit card where you get rewards for things like going to the doctor under United Healthcare Medicaid insurance available only in a few states including New York.
