If someone is a Levi, do you include it in the name for a refuah? Pardon my ignorance.
So is it just Name ben Mother's Name? or is it Name Ha Levi ben Mother's Name?

I been at a few public tehillim readings for refua for cohanim that I can think of, and no one has ever said X hakohen ben Y.
I think you just say the name.
(Doubt it can harm though)

I've seen it said name ha'levi ben mother's name ...
either way to be sure Hashem knows whom we are davening for

I've always learned that we don't use any titles when davening for a choleh. No HaCohen or HaLevi and definitely no HaRav etc.
