The only places I know of is Nordstrom's or Nordstrom's Rack. I have a friend who wears size 3 shoes, and that's the only place she can shop.
I'm sure there are specialty stores online, but then you have to deal with returns if they don't fit. Good luck!

Nordstrom online carries size 4. Call your local store if you'd rather shop in person.
Also try Zappos.
