Sick vs. tantrum - how to tell?

My oldest is 20 months old.

He's usually a really good kid, but when he's sick, he can get really fussy.

About 2 weeks ago he got a bad cold. He was really fussy. He's been getting better and better. However, on Thursday and twice on Shabbos, he starting crying and screaming for no clear reason. He seemed fine before that and suddenly became inconsolable.

He yelled, "Water" and "Bottle" but when I gave him his water and bottle, he tossed them on the floor. He usually loves pacifiers, but when I gave him his, he threw them on the floor. He did this for a good 10-15 minutes and I had absolutley no idea how to console him or what to do.

Would his behavior be a tantrum? I feel like it isn't, because it wasn't in response to anything. It's not like I said no or took something away and he freaked out.

Is this from his cold? He seemed to be getting over that. Does it sound like he's sick from something else?

Any help would be appreciated.
