An AMA for what? The doctor didn't make her pregnant and no doctor would suggest that a pregnant woman shouldn't have medical care, whether or not he approved of the pregnancy.

You can have someone sign an AMA form that they are going to care for you but that you understand that they informed you of the grave risks of future pregnancies and you got pregnant anyway. It's a tough spot to be in - OB's have to protect themselves but yes, what are you going to do? She has to get medical care.
