I am leaving my job for another job and I have a little over a week left. Though in the beginning my boss and I butted heads, lately she has been very helpful and supportive to me, both personally and professionally. Is it inappropriate to give her a small give as a sign of my hakaras hatov, or would a card be more appropriate? If I give her a gift, what kind of gift would be nice?

Inasmuch as you are leaving, a parting gift would not be construed as bribery and would be a very nice gesture on your last day there. Either a card or a gift would be appropriate and most likely completely unexpected. Flowers are always acceptable as long as she's not allergic, or else a small foliage plant if plants are permitted in the workplace. (With a plant you can say s/t corny like "thanks for helping me grow as a whateveryourprofessionis").
or something office-like, a personal planner, a desk thingie for clips, pens, scissors, picture frame if she displays pics in her office. a thermos or insulated mug if she's a big coffee/tea drinker.
I'd avoid anything personal like perfume.
