Hi to all,
I teach in high school and need to develop a sheet which contains 4 boxes for the teachers to fill in after every single class they give.
Every day there will be 1 sheet on the teacher's table, and each of the 4 teachers who teach that day will need to fill in what he/she taught.
What title can I give this sheet?
Please, I need it in English but if you know it in Hebrew Ill greatly appreciate it!

In secretary's terms, that'd be a chrono log.

Im not american so what is "chrono log"?

chrono - is short for chronological. (daily). yom yom
a log is a (sort of) list - cheshbon; din v'cheshbo - d"uch

In my class it's call the "Daily Log" but I have no idea how that would be translated. It's from when the captain of a ship used to write in every day to record what his coordinates were and what he saw.
