depending on situation you might be eligible to sign up with chai lifeline. but if you have medicaid then you can get transportation with them as posted above.
there is a bikur cholim van that leaves Boro park to manhattan at 10:00 every morn.
call 718-854-2500 for more info.
chesed is not so predictable as they don't always have a driver going to manhattan exactly the time you need.

Bikur Cholim of flatbush used to arrange rides for people. They may still do that.
