Written by R' Meshulam ben Kalonymous- born in Italy in the 10th century and later from Mainz in Germany. He wrote several of the piyutim that we say during chazarat hashatz of Shacharit (and a number in Mussaf too, I think) on Yom Kippur. This piyut is based on older, mystical sources from Eretz Yisrael in Late Antiquity. The melody is indeed an old Chabad tune.
Ha'aderet ve He'emunah is one of my favorites too
Another is Asher Ometz - also attributed to him.

Yes. It is a chabad tune thought the link you sent is an adapted version..not the original tune. I like the original better.
