So my daughter was diagnosed with pandas about 2 weeks ago and the way it came out was with panic attacks and anxiety. She has improved a lot is meant areas but she still has issues sleeping at night and some strong fears about anything related to the bathroom. Showers, using toilet etc. I took her to a therapist who gave us several tips and techniques which helped to a degree but we kind of got stuck making more progress now. Any suggestions or advice will be much appreciated.

She is on Zithromax as well.

She needs to see a CBT therapist. There are manualized programs that take 16 weeks for children but first the therapist has to spend weeks building a rapport and then once done they might start again until the child is able to use skills on their own.

Maybe we caught it really early or maybe my daughter just really wants to help herself but my daughter was comfortable at first visit to be open to the help and she meaning therapist helped by giving us ways to calm down when she's really anxious about things and they help in the moment but my daughter isn't carrying over to help prevent the next time. I.e. when she's actually in the bathroom feeling anxious we do some of the things and she gets through that bathroom visit but the anxiety and fear is there all over again the next time she needs the bathroom and we need to spend a lot of time going through the motions again and I need to stand in doorways of bathroom so she'll actually use it.

Also although I'm nor a therapist I guess I have a knack for this cuz I've been talking to my daughter so much about things and dealing that therapist told me I laid out a lot of groundwork before even first visit which cuts down a lot of time. I'm just having an issue getting her to realize she's making progress and therefore needs to be brave to try more things and not panic if first minute she's very anxious. That's where she gets scared as soon as the feeling starts feeling anxious she banks out of said activity

Are you familiar with pandas? It requires a multi prong approach. Antibiotics for the actual infection and therapy for the behavioral impact.

DID You go to a specialist for pandas or it’s your pediatrician who prescribed? My daughter had pandas it was also severe anxiety , hers was of food and eating and being alone and taking showers.... she developed tics. we went to Schulman , we had to monitor her fears and progress every few weeks by having her rate them. After 2 months I realized there wasn’t much progress on the antibiotics anymore so she weaned her. We eventually needed to see a psychiatrist because her fear of eating got so bad she was losing a lot of weight. I didn’t want to get my child medicated but I had no choice at that point. Bh she’s doing a lot better . And we did go for therapy for a long time. My child didn’t want to be in therapy so it Didnt help much, but hopefully one day she’ll realize how much the therapy can help her and she’ll be willing to go back. Hatzlacha in all you do. It’s a hard road, Hashem would give you the kochos

So antibiotics didn’t help and neither did therapy?

It doesn't. She needs the antibiotics to get better and also needs skills in dealing with the anxiety while she's recovering. PANDAS can take years to treat. You think it's fair to leave her engulfed in her anxiety each time she has a flare up? That would be cruel. She needs meds and she also needs to know how to help herself.

You need ongoing therapy along with antibiotics. The same way the antibiotics is not a one time deal, neither is the therapy.

Imho Other somatic based therapies like -sand tray therapy or somatic experiencing , Senserimotor will be far more effective than CBT. ( later cbt skills can help!)

Also physical exercise - gymnastics/ karate/ swimming/ a hobby knitting /crocheting/ art
All will help with anxiety.

And getting help for yourselves as parents. Parents dealing with PANDAS/PANS children is a huge nisoyan and getting their own support is vital !!

Im the op. I never thought to leave her engulfed in anxiety when she has a flare up. that was a different poster asking. I'm trying to get her all the help I can.

The same skills used for children with OCD will be helpful to those with pandas. You really need a therapist who is experienced with children with OCD (nowadays they are all familiar with pandas as well).

BY Us the antibiotics helped but not fully. And SChulman felt I needed a psychiatrist as well to help her because of the dangers of her not eating . Those were ocd thoughts about the food... I would suggest you call relief, Which is a mental health referral agency , they helped us find the right therapist. Just be ready to pay out of pocket. Usually the good ones don’t take insurance:

OP getting the inflammation under control will do a lot more for your child than any amount of therapy. Ibuprophen is a great first step to addressing inflammation. There are many others, both allopathic and alternative, pharma and natural. I would list them here, but honestly, imamother is not the best place for this discussion. If you are on FB, I urge you to join a pandas group. All the ones I'm on full of amazing information.
