In refaenu in shemoneh esreh when you add names into the yehi ratzon, do you add your own if you're sick? Do you add anything to it, like "li" or something, or just "lecholah Leah Dina bas Rivkah" or whatever your name is?
What about when davening for your kids? Do you add "bni" or something or just "Shmuel ben Leah Dina"?

Yes, I have said the yehi ratzon for refua sheleima for myself, using my Hebrew name. I also daven to Hashem in English - use your own words.
