The rabbi's of one of the shuls that we are a members of sent as very nice email wishing us a ksiva v'chasima tova. So what's the issue?
The greeting was
Dear Dr. Goldberg and Fruma Sara

This irked me. I don't call him by his first name. Why does dh get a term of respect and he uses my first name- its so weird to me. It's not like he forgot my last name...He is the Rav of our shul and we are not cousins, siblings, pals.. the greetings do not match. If he called my husband by his first name and me by first name ok- still weird- but ok. We are not friends from growing up etc.
Am I off base here?

I think this was clumsy. I wonder if it was automated, I've seen other clumsy labels, etc.

Could even be from a time you filled out a form or something.

Definitely automated. We write tzedaka checks to different organizations, some whose first language is not English. We receive funny mailings, letters addressed to Mr. Fruma Sarah Friedman, or other strange variations of our names as they appear on a check.
If your Rabbi, when speaking to you in person, calls your husband Dr Friedman and you Fruma Sarah, that would be very strange.

The first thought that comes to mind is that it's an automated mailing list. It definitely seems like a merging error.
