My baby has a (minor bh! ) congenital health issue and after a few specialist visits as a newborn, now needs to see a specialist once a year for a monitoring of the issue.
The doctor who first diagnosed the issue and did the initial follow up visit has now moved to a different office location, about an hour long drive each way. Now we haven't seen this doctor very many times, but he's the only one we've dealt with regarding this health issue and we were very happy with his care and bedside manner.
My child is due for a follow up visit and I have 3 options:
See the original provider even though it's a far drive
See a new provider in the old office near me (so no need to transfer documents, repeat medical history, etc)
Switch to a different provider, not part of this group

If its only once a year I would see the original doctor.

Depends if you think original dr will remember!
If dr will just read his note and there's another competent dr in practice nearby I wouldn't shlep far!
(How many times did you see him? When was the last time you saw your original dr?)
