How much do men's and women's mikvas cost in your area per dunk?
And for the men's, is there a monthly or yearly pass and if so how much is it? For the women's, is there a yearly pass and if so how much is it?
Also does the price vary by mikva within your area, and if so what does that depend on?

I do a full prep at the mikva - use the bath, use their towel, soap and sponge.
And it costs me 40 shekels (37 if I pay by credit card).
That equivalent to ~ $11.
I think just a dip is 25 NIS.
Not sure how much the men pay - I think it's somewhere between 5 - 15 NIS.

It all depends where u live. In USA mikvas are much more expensive than in israel and Europe.
In England I pay £12
Dh pays £ 18 a month . (He goes almost daily )

I'm monsey I pay $28, no matter what I do, shower or bath. I was recently in Israel where I paid so little, it was a joke. I felt bad and tipped the mikva lady since I came late (miscommunication, I was told to come whenever I want)
Dh doesn't go to the mikva

Women's mikva - 15 NIS for tevila/ shower, 30 NIS if you take a bath.
Men's - 10 NIS per tevila, 100 NIS for a monthly pass.
When I got married and lived in Jerusalem it was 20 NIS for tevila and shower, 30 NIS for tevila and bath, men's mikva was five NIS (I think) per tevila and 80 NIS per month.
The other mikva I went to in Jerusalem charged 8 NIS per tevila and 100 per month, and for women 28 NIS per tevila and another 4 NIS if you used a towel. That was a private mikva, though - I think the rabbanut mikvas in a given area all have a set price.

$20, whether it is a shower or bath. Inclusive of everything (towel or anything you may have forgotten at home.)The charge doesn't cover their operating costs.
It is a beautiful Mikvah (10 years old) and tzedaka proudly supporter by the community. Last week was their dinner fundraiser.
If someone can't afford, they would NEVER be embarrassed or turned away.
Oh, and I have never had a not-kind ML. Some are warmer than others, or may ask the senior ML if she is has question about something, but they are all discrete, professional, and gentle.
Men in our community really only go Erev YK. No idea the cost. There is a local Chabad men's only Mikvah, which I know nothing about.

Midwest. $20 for women or $18 if you were part of the new building campaign about 20 years ago. Fully stocked, 2 sizes of robes in every room.

$20 in Lakewood. By us the men only go Erev Yom Kippur, I remember giving my son a few dollars, somewhere between $3-5.
