I don't like poems as I feel it isn't personal kind of. Any ideas what to write? Something along the lines of - we appreciate your dedication and.... .thank u so much etc...

Dear teacher,
thank you so much for all that you do for Sara. We have so much hakaras hatov for all of your hard work and effort that is put in to develop all of your lessons, which sara really enjoys! Sara is lucky to have you as a teacher !

How nice of you to write a card! Extra points for not writing in rhyme.
If you can mention something specific that this teacher did - either specifically for your child or just a creative assignment for the class - that would lift your card from a generic thank you to a heartfelt expression of thanks.
