Hospital transfer questions

If someone lands up in a hospital because they couldn’t get to hospital of their choice due to emergency nature of admission and current location but has a treating Dr in a different institution, can the patient be transferred?

Yes. In most cases if patient is stable, and regular hospital is willing to accept the patient, you can do the transfer.

depending on the diagnosis and treatment options of admitting facility. Reach out to organizations such as Refuah Helpline 845.782.2000 or Chaim Medical 845.492.4917 if you are in he Tri State area. These organizations can assist with transfers if its doable.

Hospital sw here. You gotta get an accepting doctor. It sounds like you have that. Reach out to the transfer center of the hospital you’re in, say I want to be transferred to hospital B, Dr X is the accepting doctor. They have to then check insurance and bed availability . There’s no sure things but it sounds reasonable that it could be done

Thanks. Bh it’s theoretical atm. But we had to see a specialist out of state last year and had an emergency otw home. Bh patient was stable enough and we were able make it back to our usual hospital. I kept on checking where the closest hospital is in case we have to pull over... we may need to go again out of state & want to be prepared. Because the last thing after an emergency admission is to be in a hospital where this condition is unknown & not have our team.

I was able to transfer my child out of the NICU of the hospital I gave birth in to a different hospital that was more convenient for me. You need a consenting doctor in the hospital to be willing to take on the case. An ambulance transfer may not be covered by insurance though.

Logistics would be the issue - assuming the patient is deemed medically stable enough for transfer.

How are costs going to be covered in a hospital that is out of state. Most insurance companies do not cover medical expenses for personnel and facilities that are outside the state you have insurance. You should check with your current insurance company to determine what your coverage would be - this would be true even if you wanted to use the hospital without a transfer - e.g. started at the out of state facility.

Thanks everyone. Really hoping it won’t be needed. But just calms me down when I think of last years anxiety enroute & if we need to stop at a no name hospital we can beh be transferred...

I work in a transfer center at a large hospital system. In general, this is the process:
Doctor from originating hospital contacts receiving location.
Transfer request will be categorized as a family request, higher level of care , or other
If it is a family request, the insurance may not pay for the transport or may cause billing issues for the two hospitals. Case management would be involved in this case.
Once there is an accepting physician and a bed assignment (sometimes also insurance approval, see above) the transport is set up.
That may be via air or ground, nurse and monitor, or BLS based on clinical need.
Report is given from the sending RN to receiving RN, and pertinent clinical information and diagnostics are usually sent along as well.

In general, delays can be related to insurance in a case where it isn't clear that the receiving facility is a higher level of care (in NYC, you may want your child not to be at Bellevue, but Bellevue may be a higher or equal level of care). There may not be an available bed that is appropriate for the patient (a child with seizures who needs to go to CHOP may need to wait for a 9th floor bed, or one on 4ECSH EMU, to open up if video EEG may be needed). Also transfer requests are triaged, and if there are patients who have more pressing needs for the same bed or are in an unsafe location (a child needs a PICU bed in a hospital without a PICU would be prioritized over a PICU to PICU transfer).

My kid always requires a PICU bed so I guess that can make it easier if we ch’v need to transfer if landing up in some small hospital...
