Even if I use their Hebrew names, I put the official version of it, not the way they're called. For example: If a kid's name is Sury, I would put Sarah. For Rivky, I put Rivka. So I don't see the big deal if someone has a difficult name, to put the English version of it.
Also my kids who have two names only got their first name on their birth certificate because it's a huge hassle to remember that middle initial on all legal signatures and documents.
