Sounds like my daughter. There was no sudden onset, no tics, no OCD. After too many years of different therapies, parenting help, etc. turned out it was PANDAS.
I'm in Lakewood too. Who's your pediatrician?

Dr. Eilenberg and PA Beren

Can you tell me more about pandas? How is it diagnosed? What are some symptoms? Afaik my child never had strep.

They are somewhat knowledgeable about pandas. Definitely more than any other pediatrician in Lakewood. It's a clinical diagnosis but I would ask them for bloodwork to see what, if any, infections might be present.
My child was diagnose at age 6 and only had strep once in her life about 3-4 yrs earlier.

Pandas is clinical diagnosis. It involves neuropsych symptoms caused by rogue autoantibodies after infection or virus attacking brain (like covid causing neuro issues etc). Depending where the brain is inflamed, that is where ur symptoms will show. Some kids have movement issues (motor skills, tics etc), some have emotional regulation issues, some have food issues etc...
You can watch my kid is not crazy on Amazon prime and see if anything looks familiar.
Pediatrician can swab for strep and do labs for strep titers, mycoplasma and lyme that could be triggering the brain inflammation.
Kids with pandas dont usually show typical signs of infection or virus. Rather it comes with uptick of their symptoms (my son becomes psychotic when he has cold or flu...)
Lots of info on pandasnetwork.org
