We did this one year! Our older daughter dressed up as a butterfly and we dressed our infant in a caterpillar costume. Food was easy because there were so many choices from the book itself.

Sour jelly worms
Butterflies bag with popcorn or candy clipped in mid center with clothespin
The food from book
Colored sour balls stuck together as caterpillar
