We do it with a form online. Ask what your specific rabbi has set up.

Its not considered ideal, but last year we all sold chametz on line.
Ideally you should do it in person so that the Rav can make a symbolic kinyan of the chametz. If you can't then on line options are valid.

I have always sold mine online, either to my shul, or to Chabad. I give an $18.00 donation.
I print out the confirmation and tape it to the front of one of my cabinets, in a conspicuous place.
