I don't think this is possible. How many times does Shabbos fall out on a yom tov?
it could be on Rosh Hashana, once for Sukkos, once for Pesach and once for Shavuos. thats maximum 4 times.

Lehadlik ner shel yom hakipurim is said only once a year.
Nachem in Mincha on Tisha bav is also said once a year.

Nachem in Mincha on tisha ba'v is the 3rd correct answer. There's one more. Neila is not a bracha.

You have to qualify that you're talking Chutz L'aretz. Because in EY, you only say Al achilas matza, Al achilas maror, and gaal Yisroel on Pesach once a year.
