I lived in Baltimore for over 20 years. Did Baltimore to Lakewood, Baltimore to Brooklyn, Baltimore to Manhattan, there and back in 1 day, ALL the time.
Yes, it's hard if you're not used to it but it's very very doable.

you can pick up amtrak in trenton. it's an easy 30 minute drive.

Search for routes from TRE to BAL on the Amtrak website. This could be a good solution if your appointment is downtown. If not, between driving to Trenton, the train, and then taking an Uber to a different area, you won't be saving any time compared to driving directly (although if you are not used to driving long distance the train may be easier physically).

If your appointment is at Mercy medical, then the train is a really easy option. You can even walk from Baltimore Penn Station to Mercy, but I think the neighborhood is a bit rundown and sketch (someone else can confirm, though). Or take a cab ride to the hospital and back-- it's probably less than 10 minutes. .
No matter if you take the train or drive, I would definetly make sure you have someone to meet your kids at pickup. Even on trains, there can be mishaps. Less likely than driving, but it happens.
And yes, my husband always wonders why I don't want to do certain things that he thinks are so basic, so I get you on that!
although I am not a tax professional, note that either way, the trip from lakewood to baltimore and back (either the tickets or gas/tolls) count as a medical expense for tax purposes. So good to keep that in mind. If you have a flex spending account, you can likely get reimbursed for the cost of tickets. (don't quote me on that, though)
