She used to be on Coney Island, somewhere around ave L. Is she still there? I heard she may have moved. If she moved, what is her address? Anyone knows her hours? Is she by appointment or walk in?

She moved. Her new address is 1100 coney island Avenue. I don't know her hours but you don't need an appointment can just walk in.
