I think if you are asking in a complementary way " wow you lost so much kol hakavod what did you do? " is ok for friend and acquaintance ... for a friend just ask

If you are my friend, I'd be happy to share. Other people wouldn't like it.

Yes, it's absolutely personal. I'd freely compliment someone on how good she looks but not ask for specifics unless she volunteers.

It is personal, but if you're truly interested, it's totally fine to ask.

Just don't critique it in any way. Only compliment.

I have a feeling it became a more inappropriate question since surgical options have become available.

Say you met an acquaintance and said 'Wow, you look great, what did you do?!' she stammers and says 'Um, I had surgery'. That would be a real ouch situation which would never be uncomfortable had she answered 'Oh, I joined Weight Watchers'.

Of course if it's a close friend and you know she was on a diet plan then there's no harm in asking which diet plan it was.
