Is there anyone who did this in college that can help me?
My profesor said we should create a plan based on this but I cant seem to find info on the internet...

I didn't get a degree in social work but I have done such work before... (Related field).
Basically in short it is an assessment based on different aspects of your client's life. You can include everything possibly related to their situation here.
biological-age, physical/medical conditions,
Psychological
Social- education, social interferences or causes for their situation (a child whose parents are divorcing is an example).
