OP, we were by the GI today, but we are few steps before you, We only now starting Miralax, My DD is toilet trained just for few month, I thing your situation is way more complicated if it last for so long,
One more thing she recomended was giving her a time frame that she must sit on the toilet seat, not to make just to get used to it.

Having been in a similar position to you, can I recommend trying a product called Constipation Ease? I was at my wits end b/c I had 2 children that werent going and driving me nuts. After 2 different GIs and a number of different laxatives, this stuff worked like miracles! My kids were totally trained a short time afterwards, no more fear. Its all natural and non-habit forming like other laxatives and doesn't cause cramping or gas.
