Chava Rena taught me to drive. (phone number in that thread) she was extremely difficult to book but I learned so much more from her than from the random instructor I booked when I was unable to get her. she really is fantastic and I passed my test on the first try after only 4 lessons.
I've also heard great things about Devoiry Benisch but I can't tell you anything from personal experience.
