It may depend on location. In NJ, no. My daughter received EI and when she was older other therapy through a non-Jewish agency. No one ever asked to see her vaccine records.

So how should I respond to them when they claim its necessary for application? Child is on delayed schedule and don't want to bombard with all the missing ones now

If your child is on a delayed schedule, ask your doctor for a note explaining this. If it is a medical reason, it is no problem. If it is due to your choice, you can schedule your next appointment and have your doctor write that you are scheduled for vaccines on that date.

no. every state program will badger you for immunizations including wic, pre-k, etc. but legally they need to provide you service w/o it.

If you're in Brooklyn I can discuss with you in person, not online.

FYI: Most PTs are private practitioners. They don't have to see anyone they don't want to see. They shouldl be good with a doctors' note
