My insurance doesn't require a referral. I just go on my own to specialists.
If your insurance requires a referral, just call the pediatrician and tell the office staff the name of the doctor you need a referral to. They should be able to fax it to you.

Number one is he/she on a good schedule? Kids that age need 3 balanced meals and 2 healthy snacks. Treats should be just on occasion. Kids growth rate slows after age 1 I think so it's normal for a decline in appetite. What do you mean by fighting. Fighting is 2 ways so do you mean that you pressure him to eat things he doesn't want and he resists? It can be helpful to refrain from doing that as it disrupts the natural intuition that all of us are born with as to when we are hungry/full. Just continue offering 3 meals, 2 snacks, but do not react AT ALL about how much/what he chooses to eat. Can't hurt to check it out with doctor if long period of time with very little eating though. Also, are you offering a variety of foods so child can develop a taste for different flavors and textures? For example

deli, meat, chicken, fish
Eggs, cottage cheese, sliced cheese, yogurt
milk, water
chickpeas, lentils, beans, vegies, fruits
bread, wraps, rolls, bagels, cereal, oatmeal

I would recommend Dr. Joseph Levi- NYU. only it takes months to get an initial appointment.

I would second visiting GI Dr.- could very well be a food sensitivity...or nothing! My son wouldn't eat for periods of time, but it was due to having undiagnosed celiac. No need to jump to conclusions, but just putting it out there. My son was a few months older when diagnosed and had other symptoms as well

Distended stomach, irritability, diarrhea/ specific looking (white)stools (sorry if TMI). Many celiacs have very little symptoms, my son happened to have a lot

My dd did this around age 2 (stopped eating lots of food after she had been eating it). Turned out she had mono which was suppressing her appetite, but she had zero other symptoms. She went back to her normal eating (normal for a toddler anyway) afterwards.

Anyway, the most important things to check for with blood tests (from our experience):
Iron and B12 - anemia can cause decreased appetite
Mono and CMV - affect appetite, usually have no other symptoms in toddlers
Liver enzymes - also affect appetite, often a symptom of Mono / CMV / or other infection
Celiac tests

Reflux is also important to check for, as other GI issues. A gastroenterologist appt is a must.

Most importantly, track his weight!! If he's losing weight, that's more serious than if he's still gaining.

Lastly, in the meantime, don't fight about food, just make sure it's available.

I think everyone is getting way ahead of themselves with blood tests and GI doctors. The OP should start with a pediatrician, and go from there.

Update

Ds is now almost 2 yrs old. He still refuses to eat. He has one slice of bread for breakfast and a bottle before nap time. The rest of the day he begs for food and refuses to eat at the same time. He also takes a bottle at night and this is basically where he gets his nutrition from. No doctor or GI is concerned because “on the charts” his weight is fine. I’m going off my mind hearing him cry and standing by the nosh cabinet looking for food that he will eat. Any ideas? I’m desperate!

So when DD needed calories but she wasn’t interested in eating we had her drink a nesquik every day. Lots of calories, vitamins and protein. Will he go for chocolate milk?

Hugs. This sounds really hard. Unfortunately I don't have ideas for you. I just want to say though that this doesn't sound regular at all - please trust your gut rather than listening to these doctors. I'm bother more about him crying and begging for food than the fact he isn't eating. You need to get the root of the problem, it sounds like he's really suffering.

Hashem should help you find the right shaliach soon.

I can definitely try, but:
1- he gets bored of everything after 3 days, so probably not a long-term solution
2- is that enough for him to be full from? Basically it means he will only be drinking all day

Thanks for giving of your time to try to help, I’m just really confused. If I give him just 1 bottle and 1 cup choc milk, how does that fill him up so he’s not hungry? He’s hungry with 2 bottles a day, cuz how long do you think a bottle keeps him going?

It’s like drinking a protein drink. It’s supposed to keep you satisfied longer. Like Boost or Ensure (which we used also).

It has more calories than just plain milk and more vitamins etc.

ETA: it worked for dd when she had no desire to eat. It couldn’t hurt to try. It might help at least until you can figure out something else.

It seams that I awasnt specific. Sorry. He takes 2 FORMULA bottles a day. The GI said it doesn’t fill him up enough to be the reason for him being uninterested in food, but maybe sensory. I still don’t know what to do about that.... he gets OT, but not helping yet
